A male Brazilian Butt Lift is the same operation as its female counterpart and almost the opposite plan. Fat is harvested by liposuction, purified and re-injected into the subcutaneous plane of the buttocks — but where a female BBL adds projection and roundness, a male BBL builds a squarer, flatter, more lateral shape: filling the hip dips, lifting the upper buttock, and sharpening the taper from the back down to the waist.

The male aesthetic goal is not a curve. It is a continuous line from the latissimus down to the glute, with no hollow at the side of the hip. Volume is placed high and laterally, in modest quantity, so the torso keeps its V-shape. Over-injection, or volume placed low and central, is what makes a male result read as feminine — and it is very difficult to undo.
Men carry less subcutaneous fat than women, and what they carry is more fibrous and harder to harvest atraumatically. Two liposuction areas are rarely enough. Most male patients need Lipo 360 — abdomen, flanks, lower and upper back — which also happens to be exactly the treatment that produces the taper the result depends on.
Below a certain body-fat percentage there is not enough graft material, and injecting it anyway produces a thin, short-lived result. In that case we either recommend gluteal implants or ask you to gain a controlled amount of weight before surgery. We would rather tell you at the consultation than operate on insufficient donor fat.

The fat is placed higher and more laterally, and far less volume is injected. Using the female template on a man is the single most common way this operation goes wrong.

Men carry less subcutaneous fat than women and it is more fibrous, so donor sites are almost always the full abdomen, flanks, lower and upper back rather than two areas. That is why the male procedure is normally quoted with Lipo 360. We assess donor volume during your free consultation before committing to a plan.
